Checklist for Applying to the Single Subject Credentialing Program in English

Fresh Voices 2009-2010: Release Form
To submit your English 134 essays and artwork for publication consideration
in Fresh Voices 2009-2010, sign this release form and give it, along
with a hard copy of your essay, to your ENGL 134 instructor. In addition, you
must email your submission(s) to engl-freshvoices@calpoly.edu.
All submissions must be received by June 12, 2009.
